At the time of your initial visit, bring your completed downloaded forms with you. If you have not downloaded the forms, please arrive at least 15 minutes before your scheduled appointment time and we will supply them to you and you will be able to complete them on site.
If your referring dentist or physician has given you any records, please remember to bring them with you.
Remember to bring a list of all medications (prescription as well as over the counter) with you. If you have downloaded the form then you can fill in this information ahead of time. If you have dental or medical insurance, please bring your ID card(s).
A parent or legal guardian must accompany all our patients under the age of 18.

We will perform a comprehensive review of your medical and dental history as well as a thorough examination to measure bone loss, loose teeth, receding gums, bite irregularities, oral cancer screening, jaw joint problems (TMJ) and other signs of dental disease.
We will also provide you with a consultation where various treatment options will be presented. A written comprehensive treatment plan with associated fees may also be provided.
